Continued Dispensing Forms
After you dispense a Continued Dispensing supply, the following forms are printed along with the medication labels:
- Repeat form: Used for claiming only (repeats are not allowed for Continued Dispensing supplies).
- Consumer Declaration and Prescriber Notification form: Three copies are printed.
Continued Dispensing is written in four distinct locations on the repeat form.
The repeat form must be signed and dated by the patient, then submitted with the claim by the pharmacist.Do not hand it back to the patient.
This form is intended to inform the doctor of the details of the Continued Dispensing supply. It displays the details of the pharmacists, patient, and medication.
Three copies are printed: One for the Patient; one to send to the prescriber; and one to keep on record at the pharmacy.
: This form must be dated and signed at the time of dispensing by both the pharmacist and the patient.
The wording on the form has altered slightly. Under 'I am satisfied that:' the second bullet point now says:
The usual prescriber is unable to be contacted and/or is unable to provide an e-prescription or owing prescription
Pharmacy receipt (Optional)
The pharmacist may choose to print a pharmacy receipt for the Continued Dispensing supply. The script type (Continued Dispensing) and the pharmacist's AHPRA number are displayed at the top of the receipt.
